Alerts and callouts get icons and keep their titles in the header

GFM alerts now show an icon for their type, and a title written after the marker, as Obsidian callouts use, appears in the header instead of on a line below it.

GFM alerts (> [!NOTE], > [!TIP], > [!IMPORTANT], > [!WARNING], > [!CAUTION]) now draw an icon for their type next to the label, the way GitHub and Obsidian show them. The icons are decoration: screen readers skip them and read the label or title.

Text after the marker on the same line is now the alert's title. > [!NOTE] Before you start shows "Before you start" in the header in place of "Note". Before, Obsidian callout titles exported through the Obsidian plugin landed on a line below the label. Obsidian's fold signs (> [!NOTE]-, > [!NOTE]+) are accepted too.

Alerts without a title look as before, with the icon added. The icons do not count towards the image limits. For Obsidian callout titles to appear in the header, update the makesPDF Obsidian plugin once the new version is out. See Markdown for the full list of supported syntax.
